Objective decline with preserved independence — when to assess and how
MCI is objective cognitive impairment (typically 1–1.5 SD below norms) with preserved functional independence and no dementia. Annual conversion to dementia is 10–15% in clinic samples vs 1–2% in cognitively normal elders. Concern about change, objective impairment in one or more domains, preserved daily function, and exclusion of dementia define MCI. Amnestic MCI often heralds Alzheimer pathology; non-amnestic MCI may progress to non-AD dementias. Informant input is essential — patients often under-report.
Counselling, vascular risk optimization, sleep and mood treatment, cognitive strategies, driving and work planning, and family support are discussed. Follow-up screens for conversion and maintains caregiver wellbeing. No drug is curative; the value is early planning.
