Movement patterns evaluated
Rest tremor, action tremor, bradykinesia (slowness), rigidity, and gait/balance changes. Essential tremor is action-related and often familial; Parkinsonian tremor is at rest with slowness and stiffness. Medication, thyroid, and cerebellar causes also produce shaking.
- Rest tremor - at rest, often hand
- Action tremor - worse with movement
- Bradykinesia - slowness of movement
- Rigidity - stiffness

Evaluation approach
Detailed timing of tremor (rest vs action vs postural), family and medication review, neurological exam, and selective investigations to exclude mimics. Discussion of differential and next steps is individualized.
