What small vessel disease is
Cerebral small vessel disease refers to pathological changes in the small arteries, arterioles, capillaries, and venules in the brain. These changes include lipohyalinosis (vessel wall thickening), microatheroma, lacunar infarcts (small strokes), white matter hyperintensities, cerebral microbleeds, and perivascular spaces. Small vessel disease is a major cause of vascular cognitive impairment, gait disturbance, and lacunar stroke.
The most common risk factors are hypertension, diabetes, smoking, and aging. Hypertension is particularly important because chronic elevated blood pressure damages small vessel walls over decades. Small vessel disease often progresses silently until cognitive decline, gait problems, or stroke symptoms appear. MRI brain is the diagnostic tool of choice, showing white matter hyperintensities, lacunes, microbleeds, and enlarged perivascular spaces.

Cognitive impact and prevention
Small vessel disease contributes to vascular cognitive impairment and vascular dementia. The cognitive pattern typically involves slowed processing speed, executive dysfunction (difficulty with planning, organisation, and complex tasks), and gait disturbance rather than the prominent memory loss seen in Alzheimer's disease.
