These measurements classify mild, moderate, and severe deformity and guide whether soft-tissue balancing alone would be insufficient and whether a bony procedure is considered.</p> <h2>Conservative measures that medical consensus supports</h2> <p>For many patients with mild to moderate symptoms, non-surgical care reduces pain and slows progression without changing bony alignment. General medical consensus emphasizes:</p> <ul> <li>Footwear modification: wide, soft toe box, low heel, adequate length, and avoiding seams over the prominence. Their role is palliative, and expectations should be transparent, especially for moderate or severe deformity.</p> <h2>When surgical correction is discussed</h2> <p>Surgery is considered when pain persists despite appropriate conservative measures, functional limitation affects work or walking distance, or progressive deformity creates secondary problems such as lesser-toe overload or recurrent bursitis. It is not recommended for an asymptomatic bunion purely for appearance, as all surgery carries risk. Decision-making incorporates angle severity, joint congruency, presence of arthritis at the first metatarsophalangeal joint, patient activity demands, smoking status, diabetes, and vascular status.</p> <p>Surgical options are chosen by deformity pattern rather than by a single technique. Distal chevron or Austin osteotomy suits mild to moderate deformity with a modestly increased intermetatarsal angle. Scarf or Ludloff shaft osteotomy and proximal opening or closing wedge osteotomies address larger intermetatarsal angles. Lapidus (first tarsometatarsal fusion) is considered when hypermobility or arthritis at that joint drives recurrence. Akin phalangeal osteotomy may supplement alignment of the hallux itself, and lateral soft-tissue release with medial capsulorrhaphy balances tissues but is insufficient alone for bony deformity. Minimally invasive percutaneous techniques are an active research area; early series report pain and alignment outcomes comparable to open surgery in selected patients, but long-term comparative data are still maturing, so candidacy is selective.</p> <h2>Recovery, risks, and shared decision-making</h2> <p>Recovery protocols depend on fixation stability and osteotomy location. Many weight-bearing protocols allow heel weight-bearing in a postoperative shoe within days and progression to full weight-bearing over 3 to 6 weeks, followed by physiotherapy for gait, swelling control, and range of motion. Swelling can persist for 3 to 6 months. Evidence-based risks discussed before consent include recurrence, transfer metatarsalgia, stiffness, delayed union or nonunion, infection, nerve irritation with numbness over the medial hallux, and hardware prominence. Smoking, poorly controlled blood sugar, and peripheral vascular disease increase wound and union complications, so optimization before elective surgery matters.
